Independent Contractor/Medical Case Manager (Bilingual)

Case Manager Job Responsibilities:

Accomplishes clients' care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; Coordinate care that is safe, timely, effective, efficient and client-centered.  Helps achieve and maintain wellness and autonomy.  Promote quality and cost-effective interventions and outcomes.  Completion of comprehensive monthly reports in a timely manner.
